Position Summary &amp; Responsibilities
<ul>

Evaluate patients and establish PT treatment plans
</li>
<li>
Provide mobility, gait, transfer, and strengthening interventions
</li>
<li>
Monitor patient progress and adjust care plans
</li>
<li>
Collaborate with interdisciplinary care team
</li>
<li>
Complete accurate and timely documentation
</li>
</ul>
Requirements
<ul>
<li>
Certification in Physical Therapy
</li>
<li>
Active California PT license (required)
</li>
<li>
SNF or rehab experience preferred
</li>
<li>
Strong clinical and communication skills
</li>
