Job DescriptionWe're looking for a compassionate, resourceful Case Manager to support a 50-unit senior housing community in Buellton, CA.

This is a hands-on, on-site role where your work directly impacts people's ability to remain safely and successfully housed.

What You'll Be Doing

  • Provide individualized case management based on each resident's needs
  • Coordinate community resources (mental health, financial assistance, senior services, veteran resources, etc.)
  • Collaborate with outside caseworkers, agencies, and community partners
  • Support residents transitioning into housing after homelessness
  • Help residents navigate challenges such as health needs, housing stability, and organization/hoarding concerns
  • Support and facilitate community activities like meetings, groups, or on-site programs (gardens, classes, wellness events)

What We're Looking For

  • Bachelor's degree (required) or Master's degree (MSW, MFT, or related, preferred)
  • A minimum of 3 years of case management or related social services experience (depending on education level)
  • Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferred
  • Comfortable working independently while collaborating with external partners
  • Strong communication, organization, and follow?through skills
  • Spanish speaking is a plus, but not required

Monday-Friday, 8:30 AM-5:30 PM | 100% on-site in Buellton, CA

Available paid leave may include Paid Sick Leave, where required by law; any other paid leave required by Federal, State, or local law; and Holiday pay upon meeting eligibility criteria.

Pay Details: $62,400.00 to $72,800.00 per year
