Key duties include:

<li>Navigating the complex Medicare/Medicaid resources alongside families</li>
<li>Participating the team care planning process</li>
<li>Coordinating safe discharge plans</li>
<li>Resident and family counseling &amp; support</li>

<li>Advocacy and grievance interventions</li>
