Position Summary &amp; Responsibilities:

Evaluate, assess, and treat SNF residents
</li>
<li>
Develop and update individualized PT care plans
</li>
<li>
Provide therapeutic exercises, gait training, and mobility support
</li>
<li>
Document patient progress and outcomes per facility standards
</li>
<li>
Collaborate with interdisciplinary care team
</li>
<li>
Support safe discharge planning and functional mobility goals
</li>
