Job Description
Job DescriptionResponsible for planning, coordinating, and documenting skilled nursing care for an assigned caseload of home health patients. Utilizing physician orders, clinical judgment, and professional nursing skills, the Home Health RN, in collaboration with the Case Manager, develops and implements individualized care plans that address each patient’s specific needs while ensuring compliance with accrediting organization standards, Federal and State regulations, reimbursement guidelines, and agency policies and procedures.
The Home Health RN provides comprehensive skilled nursing services, including patient assessments, medication management, disease management, wound care management, patient education, and coordination of care with the interdisciplinary team. Participates in Quality Assessment and Performance Improvement (QAPI) activities to promote continuous improvement in patient outcomes and quality of care.
Duties:
- Performing initial evaluation visits, including OASIS Start of Care (SOC) assessments
- Conducting comprehensive patient assessments and regularly re-evaluating nursing needs
- Developing, initiating, and revising individualized plans of care based on patient needs and physician orders
- Providing skilled nursing services requiring substantial and specialized nursing judgment
- Performing wound assessments, wound care treatments, and monitoring patient progress according to physician orders and evidence-based practices
- Initiating appropriate preventive and rehabilitative nursing interventions
- Managing patients with complex medical conditions and providing education related to disease management, medications, treatments, and preventative care
- Preparing accurate clinical documentation, including visit notes, assessments, and progress updates
- Coordinating care with physicians, case managers, therapists, and other members of the interdisciplinary team
- Communicating changes in patient condition, needs, and progress to physicians and appropriate personnel
- Educating and counseling patients and families regarding health management, safety, and ongoing care needs
- Participating in in-service education, mentoring, and supervising other nursing personnel as needed
- Participating in agency Quality Assessment and Performance Improvement (QAPI) activities
Regulatory Requirements:
- Current Registered Nurse (RN) license in the State of California
- Current BLS/CPR certification
- One (1) year of acute care nursing or skilled nursing experience preferred
- Prior home health experience preferred; however, new graduates with strong clinical skills and a willingness to learn are encouraged to apply
- Wound care experience preferred
- Specialty certifications or skills preferred (i.e., phlebotomy, IV therapy, enteral therapy, wound care certifications)
- Valid California Driver’s License, current automobile insurance, and reliable transportation
Language Skills:
- Ability to communicate effectively in English, both verbally and in writing
- Additional language skills preferred
Skills:
- Proficiency with electronic medical records (EMR) systems
- Knowledge of current Federal, State, accreditation, and agency regulations related to home healthcare
- Strong clinical assessment, communication, organization, and documentation skills
- Ability to provide compassionate, patient-centered care in a home-based environment
