A Pharmacist opportunity is available in a Long-Term Acute Care (LTAC) facility with 50-60 beds, located in Kentfield, CA. This position is part-time with the possibility of transitioning to full-time and offers day shift hours. The facility utilizes Wellsky and PYXIS automation systems.
Key Responsibilities:
- Provide pharmaceutical care in the LTAC setting, ensuring safe and effective medication use.
- Monitor pharmacokinetics and manage anticoagulation therapy as needed.
- Collaborate with healthcare professionals to manage inpatient pharmaceutical needs, including TPN.
- Maintain clinical knowledge and apply it to patient care in an acute care environment.
Qualifications and Desired Experience:
- Bachelor of Science Degree from an accredited College/School of Pharmacy required; Doctor of Pharmacy degree preferred.
- Active California Pharmacist License.
- Inpatient or hospital pharmacy experience preferred.
- Pharmacokinetics and anticoagulation experience preferred.
- Experience with Total Parenteral Nutrition (TPN) preferred.
- Current BLS and ACLS certifications from an approved vendor preferred.
- Candidates with 2-3 years of retail pharmacy experience interested in maintaining clinical skills in an acute care setting are encouraged to apply.
- Candidates with more than 10 years of community pharmacy experience or new PGY1 residency graduates are less aligned with this role.
Schedule:
- Part-time days, with potential to increase to full-time.
