Job Overview
We are seeking an experienced Registered Nurse (RN) – Case Manager for a 13-week assignment at a leading acute care hospital in Mission Viejo, CA. This role is ideal for RNs with a strong background in acute care case management and care coordination.
Responsibilities
- Coordinate patient care and discharge planning.
- Collaborate with physicians and the interdisciplinary healthcare team.
- Assess patient needs and arrange appropriate post-acute services.
- Ensure continuity of care and timely discharge planning.
- Maintain accurate documentation and case management records.
- Advocate for quality, patient-centered care.
Requirements
- Active California RN License
- Minimum 2 years of recent acute care Case Management/Care Coordination experience.
- Previous travel nursing experience is required.
- Strong communication, organizational, and care coordination skills.
- Willingness to float to the other facility as needed.
- Availability for weekend rotation.
