Location: Napa, California

Schedule: Day shift, 5x8-hour shifts (08:00 - 17:00), 40 hours per week. No weekends or on-call required. No floating within the assignment.
Highlights:\n

- Licensed Adult Day Health program serving frail elderly, younger disabled adults, and individuals with Alzheimer's Disease and related dementia
- Provide chronic disease case management for an average daily census of 72 participants in a program licensed for 100 per day
- Collaborate with an interdisciplinary team including physical therapy, social services, interpretation services, and transportation support
- Focus on reducing emergency department visits and hospitalizations while preventing premature nursing home placement
- Patient ratio of 1:40 with approximately 6 visits per day; work with conditions including diabetes, hypertension, CHF, COPD, and degenerative neurological diseases
- First-time travelers accepted; local candidates welcome

Requirements:\n
- \n
- Active California RN license required at time of start
- Minimum 1 year of RN experience required; clinic, primary care, pre/post-operative, or home health experience preferred
- Current BLS certification and First Aid required
- Experience with care coordination, disease management, and plan of care development
- Knowledge of HIPAA guidelines and ability to determine medical necessity per evidence-based guidelines
- Reliable transportation required; free parking provided
- Two professional references covering 1 year within the last 3 years
