Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting across an assigned territory
- Evaluate, plan, and implement individualized treatment plans to meet patient goals and functional needs
- Meet a productivity standard of 30 points per week
- Document all patient care accurately and in a timely manner using the HCHB electronic medical records system
- Participate in on-call and weekend coverage as required
- Collaborate with the interdisciplinary care team to ensure coordinated and effective patient outcomes
Required Qualifications
- Active Physical Therapist (PT) license in the state of California
- Minimum of 1 year of home health experience with a Medicare-certified facility
- Proficiency with HCHB (Homecare Homebase) EMR system required
- Ability to meet weekly productivity expectations of 30 points per week
- Availability to work Sunday–Thursday and participate in on-call and weekend requirements
Preferred Experience / Skills
- Familiarity with the service territory including Capistrano Beach, Rancho Mission Viejo, Ladera Ranch, San Clemente, and San Juan Capistrano
- Strong time management and organizational skills suited for an independent, field-based work environment
