Shift: Day 5x8-Hour (08:00 - 16:30)
Job Summary
We are seeking an experienced Medical Social Worker (MSW) to join our Acute Care Case Management team. The ideal candidate will have at least 2 years of acute care hospital case management/discharge planning experience and be proficient in psychosocial assessments, discharge planning, care coordination, and connecting patients with post-acute and community resources. This role requires close collaboration with physicians, nurses, utilization management, insurance case managers, and multidisciplinary teams to ensure safe, timely, and cost-effective patient discharges.
Requirements and Qualifications:
· Active California professional license (required).
· Minimum 2 years of acute care case management/discharge planning hospital experience.
· Strong knowledge of discharge planning, utilization management, and care coordination.
· Excellent communication, organizational, and critical thinking skills.
· Ability to work collaboratively in a multidisciplinary acute care environment.
· Flexibility to float as needed.
· Experience using Epic EMR.
· Must be available to start ASAP.
