Responsibilities

  • Provide comprehensive occupational therapy assessments and tailored treatment plans.

  • Deliver hands-on OT interventions to improve daily living skills and functional independence.

  • Adhere to regulatory, safety, and documentation standards for compliant practice.

  • Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.

  • Monitor patient progress, adjust goals, and document outcomes in the care management system.

  • Utilize evidence-based practices and data to drive care decisions and demonstrate impact.

  • Educate patients and families on home programs, safety, and resource utilization.

  • Engage in continuous professional development and share learnings to support growth.

  • Participate in performance metrics, target setting, and pipeline planning for patient flow.

  • Identify and advocate for equipment or environmental modifications to support function.

  • Support business growth by contributing to clinic outreach and community education.


Experience/Qualifications

  • OT license and regulatory compliance for clinical practice.

  • Comprehensive OT assessments and individualized treatment plans.

  • Hands-on OT interventions to enhance ADL and functional independence.

  • Accurate documentation and adherence to safety/regulatory standards.

  • Evidence-based practice utilization and outcome-driven decision making.

  • Interdisciplinary collaboration for care coordination and discharge planning.

  • Progress monitoring, goal adjustment, and care management system updates.

  • Participation in performance metrics, pipeline planning, and patient flow targets.

  • Patient and family education on home programs, safety, and resources.

  • Equipment and environmental modification advocacy to support function.
