Immediate Supervisor: Program Director
POSITION PURPOSE
The Occupational Therapist is responsible for providing evaluation, treatment, and rehabilitation services to older adults participating in the Adult Day Health Care program. This role focuses on improving participants’ physical, cognitive, and motor skills to enhance independence and overall quality of life. The Occupational Therapist develops individualized care plans, implements therapeutic interventions, and collaborates with interdisciplinary team members to ensure comprehensive, participant-centered care.
DUTIES AND RESPONSIBILITIES
- Conduct comprehensive participant assessments to determine functional needs, strengths, and goals
- Develop and implement individualized Occupational Therapy Plans of Care (IPC)
- Provide therapeutic interventions to improve activities of daily living (ADLs) and functional independence
- Monitor and evaluate participant progress and adjust treatment plans as needed
- Educate participants and families on therapeutic techniques, adaptive strategies, and equipment use
- Collaborate with interdisciplinary team members to coordinate and support participant care
- Maintain accurate, timely, and compliant documentation of assessments, treatment plans, and progress notes
- Recommend adaptive equipment and environmental modifications to support participant safety and independence
- Participate in care planning and team meetings as required
- Ensure compliance with applicable clinical standards and program regulations
- Perform additional duties as assigned to support participant care and program operations
Requirements:
MINIMUM REQUIREMENTS
- Graduate of an accredited Occupational Therapy program
- Current and valid Occupational Therapy license in the State of California required
- Experience working with older adults or individuals with disabilities preferred
- Strong clinical assessment and treatment planning skills
KNOWLEDGE, SKILLS, AND ABILITIES
- Strong knowledge of occupational therapy principles and rehabilitation practices
- Ability to conduct functional assessments and develop individualized treatment plans
- Excellent communication and interpersonal skills
- Ability to collaborate effectively within an interdisciplinary team
- Strong documentation, organizational, and time management skills
- Ability to educate participants and families on therapeutic interventions
- Commitment to patient-centered care, dignity, and respect
- Ability to work independently while maintaining professional collaboration
