Responsibilities:

<li>Develop and execute individual treatment plans</li>
<li>Determine appropriate milestones to gauge client progress</li>

<li>Facilitate referrals to other healthcare professionals and programs</li>
<li>Maintain accurate client documentation</li>

Qualifications:
