We are seeking a motivated and skilled mid-level provider to join our outpatient endocrinology team serving South Riverside County. In this role, you will take a lead role in the assessment, treatment, and ongoing management of patients with endocrine and metabolic disorders — including diabetes, thyroid disease, adrenal conditions, and other hormonal pathologies.

You will work within a multidisciplinary team environment, rounding with a degree of clinical autonomy while collaborating closely with physicians, dietitians, and nursing staff to ensure comprehensive, coordinated patient care. Candidates with prior acute or hospital-based experience are strongly encouraged to apply.

Key Responsibilities

Clinical Assessment Perform comprehensive endocrine evaluations including detailed history, physical examination, laboratory interpretation, and risk stratification for diabetes and hormonal disorders.Treatment & Disease Management Develop individualized, evidence-based treatment plans for conditions including diabetes mellitus (Type 1 and Type 2), thyroid disorders, osteoporosis, adrenal and pituitary conditions, and metabolic syndrome.Patient Education Empower patients and their families through education on disease processes, lifestyle modifications, medication adherence, and self-management strategies.Collaborative Care Partner with physicians, dietitians, nurses, and other specialists to coordinate care, optimize outcomes, and support a team-first culture.Procedures & Documentation Perform and interpret point-of-care testing (glucose monitoring, A1C, thyroid ultrasound if trained); adjust medications accordingly. Maintain thorough, accurate clinical documentation including assessments, treatment plans, progress notes, and outcomes.Quality & Advocacy Participate in quality improvement initiatives and advocate fiercely for each patient's access to comprehensive, compassionate care.

Qualifications

Education & Certification

  • Master's degree in Nursing with Nurse Practitioner certification required
  • Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred
  • Physician Assistant candidates with equivalent credentials will be considered

Licensure

  • Current California NP or PA license
  • Active DEA registration
  • APRN certification (for NP candidates)

Experience

  • Minimum 1 year of clinical experience as a Nurse Practitioner or Physician Assistant
  • Prior exposure to endocrinology, diabetes management, or internal medicine strongly preferred
  • At least 1 year of acute or hospital-based experience preferred

Skills & Attributes

  • Strong clinical decision-making and critical thinking skills for managing complex endocrine presentations

  • Comfortable working autonomously within a collaborative team structure
  • Genuine commitment to patient-centered, compassionate care
  • Certified Diabetes Care and Education Specialist (CDCES/CDE) or endocrinology-focused training a plus, but not required
