Job Summary
The Hospice Admissions RN is responsible for evaluating patients for hospice eligibility and coordinating the admission process. This role includes completing comprehensive assessments, initiating plans of care, and supporting patients and families during the transition to hospice services. The RN works closely with physicians and the interdisciplinary team to ensure timely, compassionate, and appropriate care.
Key Responsibilities, Admissions & Patient Care
Complete initial and comprehensive hospice assessments for patients and families
Determine hospice eligibility and identify patient care needs
Initiate and develop individualized plans of care
Provide skilled nursing care in accordance with the nursing process and state regulations
Administer medications and treatments as ordered
Educate patients and families on hospice services, care plans, and symptom management
Support patients with comfort care and activities of daily living as needed
Care Coordination & Communication
Communicate effectively with physicians to obtain orders and report patient status
Coordinate care with the hospice interdisciplinary team and community resources
Maintain accurate, timely clinical documentation
Provide education and guidance to patients, families, and caregivers
Participate in interdisciplinary team meetings
Additional Responsibilities
Assist with coordination of equipment, supplies, and services needed for admission
Support continuity of care during the admission process
Participate in on-call rotation as required
Supervise and delegate tasks to hospice aides when appropriate
Engage in ongoing professional development and training
Participate in quality improvement initiatives
Qualifications
Graduate of an accredited nursing program
Current California RN license and CPR certification
Minimum of 1 year of RN experience in (hospice &home health preferred and Kinser knowledge
Strong assessment, communication, and organizational skills
Ability to work independently with minimal supervision
Valid drivers license and reliable, insured vehicle
