All Care Hospice is currently seeking a compassionate, organized, and dedicated Licensed Vocational Nurse (LVN) to join our team as a Hospice Care Coordinator in our Glendale office.
Position Requirements:

Current California LVN License (required).

Hospice Care Coordinator experience preferred.

Strong communication, organization, and time management skills.

Ability to coordinate patient care with physicians, nurses, and interdisciplinary team members.

Comfortable working in a fast-paced office environment.

Willing to make patient visits in the field when needed.

Knowledge of hospice regulations and documentation is a plus.
Schedule:

Full-Time.

Monday through Friday.
