Job DescriptionJob Title: Integrated Care Nurse PractitionerLocation: Candidates should be located in Fresno (also open to Clovis)Salary: $120,000 - $145,000Job Overview:The Advanced Practice Provider delivers personalized, value-based medical care to patients living with chronic kidney disease and end-stage renal disease. This is a field-based role providing in-home and virtual comprehensive assessments, care coordination, and preventative management. The APP works closely with field nurses, community health workers, and physicians to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life.This is not an office-based position. The provider must reside within 30 miles of Fresno, California, and be comfortable traveling within their assigned territory. The role also includes occasional travel (approximately every other month) to the San Luis Obispo or Santa Barbara areas for in-home patient visits. These trips may last up to three days (two nights). For travel requiring drives of two hours or more, approved expenses such as rental car and hotel accommodations are covered.Key Responsibilities:Conduct comprehensive annual wellness exams in the home and virtual settingsPerform health assessments and develop individualized treatment plansPrescribe medications and order diagnostic testing as appropriateEducate patients and families on disease management and preventive careCoordinate care with primary care providers, specialists, and ancillary servicesComplete documentation accurately and timely, including full history, assessment, and care planIdentify and close gaps in care through preventive monitoring and collaborationParticipate in integrated care team meetingsDeliver evidence-based care that improves quality metrics and patient satisfactionAssist patients with accessing educational and support resourcesRequired Qualifications:Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Associate licenseBoard certification (NP: ANCC or AANP; PA: NCCPA)Current and unrestricted DEA certificateMinimum of 2 years of direct patient care experienceValid driver's license, reliable transportation, and car insuranceAbility to practice autonomously without direct supervisionProficiency with electronic medical records and point-of-care technologyStrong knowledge of HIPAA, OSHA, and applicable federal and state regulationsPreferred Qualifications:Managed Care, IPA, or Health Plan experienceExperience conducting annual wellness visits in-home or virtuallyBilingual in Spanish preferredExperience in chronic kidney disease or value-based care environmentsBenefits:Competitive salaryComprehensive medical, dental, vision, and life insuranceFlexible paid leave and vacation policy401(k) with employer matchingOpportunity to work in a fast-growing, innovative, value-based kidney care organizationPandoLogic.
