Location: Elk Grove, CA
Job Type: Part-Time (20-25 Hours/Week)
Setting: Outpatient Primary Care / Family Medicine
Patient Population: Adults, Seniors, Medicare, Medi-Cal, and Commercial Insurance
About the Opportunity:
We are seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join a growing outpatient primary care practice in Elk Grove, CA. This role offers the opportunity to provide high-quality, patient-centered care in a collaborative environment focused on preventive medicine, chronic disease management, and continuity of care.
Key Responsibilities

Provide comprehensive outpatient primary care services.

Evaluate, diagnose, and treat acute and chronic medical conditions.

Perform physical examinations and patient assessments.

Order and interpret laboratory tests and diagnostic studies.

Prescribe medications in accordance with California regulations.

Conduct preventive care visits, annual wellness exams, and chronic disease management.

Educate patients on treatment plans and healthy lifestyle choices.

Coordinate specialty referrals and follow-up care.

Maintain accurate and timely EMR documentation.

Collaborate with physicians and the clinical team to deliver quality patient care.
Qualifications
Required:
-
Active California Nurse Practitioner or Physician Assistant license.

Strong clinical assessment and communication skills.
-
Comfortable using Electronic Medical Records (EMR).
Preferred:
-
Experience in Family Medicine, Primary Care, Internal Medicine, Adult Medicine, or Urgent Care.

DEA registration and BLS certification.
-
Experience with Medicare Wellness Visits, Preventive Care, Chronic Disease Management, and HEDIS/IHA quality measures.
New Graduates: Strong new graduates with primary care or outpatient clinical training are encouraged to apply.
