Position Title: SUDSpecialist Service Case Manager
ReportsTo: Substance Use DisorderProgram Director
Status: Full-Time- Union
Schedule: M-F / 8am - 5pm
Position Summary:
Clinica RomerosSubstance Use Disorder CaseManager will provide Case Management to Court/DCFS/Probation/Self-Referred clients. Willassistclients in meeting program goals andobjectives, document allprogressandfacilitategroup sessions as needed.Assistwith the outreach efforts and promote Drug and Alcohol Department Services to the community.Insurenew enrolment outcomes are being met. Oversee Speciality Services cases, all referrals, classes, and individual session/groups andinsurethat outcomes are met and reported to Program Director at all program sites.
Responsibilities:
- Provide case management and group sessions to clients experiencing chemical abuse or chemical dependency.
- Insurethat client is meeting treatment plan goals and that all documentation is beingmaintainedin client charts intime framerequired.
- Insureprogress notes, discharge summaries,intakesand all client forms are completed.
- Typical 1-2 years of experience infield.
- Skills/Specialties include Case Management and Groupfacilitation.
- Must be registered with one of the following: CADDE,CATCor CADAC.
- Responsible for recruitment of new clients in both the youth andadult population
- Ability to engage the community and have them refer Clinica Romero as their program of choice
- Keep track of all Specialty Services attendance, provide progressreportsand provide alllettersare completedregardingpatientsprogress and sent to referralsourceand or client requesting the information.
- Meet with Social Workers, Probation/Parole officers and all other individuals who have referred clients by phone, in person and through meetings.
- Case Manage/Counselboth youths and adults that have substance abuse problems (Ages 12 and up).
- Responsible fora substantialcase load and providing their clients with quality service
- Direct one on one sessions and administer crisis intervention/ casemanagement
- Facilitate and prepare groups sessions onvarious topicsrelating to recovery
- Complete all necessary documentation ina timelymanner.This includes reports, progress notes, discharge summaries, treatment plans, aftercareplansand all other required documentation.
- Insureall classes meet ontimeand curriculum isupdatesat all program sites.
- Transport clients to and from program if needed
- Meet with staff to discuss individual progress of shared patientswhoservices are being provided to.
- Provide letters for clients and referring agencies when needed for Anger Management, Domestic Violence and Parenting
- Attend meetings, communityeventsand outreach to insure we are meeting capacity for specialty services
- And other assigned duties
- Maintainall charts related to Parenting, Anger Management and Domestic Violence
Qualifications
- Must have Certification tofacilitateAngerManagement ,and Parenting and working on obtaining Certification tofacilitateDomestic Violence
- Minimum one yearfacilitatingAnger Management, Parenting and Domestic Violence
- A minimum of one year experienceworking inthe field ofSubstance Use Disorderandaddiction
- Must be highly motivated anddemonstrategood communicationskills
- Must have the ability to recruit new clients (adults and youth) to our agency.
- Must meet expected outcomes in terms of new clients attendingas a result ofyour efforts in the community.
- Must meet goals andobjectivesgiven by Program Manager.
- Musthave an understanding ofthe 12 Core Functions
- Must have the ability to write individual and group progress notes/treatment plans/ needed documentation that meet county standard
- Must have experience in group facilitation.
- Mustdemonstratean understanding of the recovery process, which includes thetwelve stepprogram(s).
- Must haveexperience casemanaging/counseling clients in recovery.
- Must have experience in observed drug testing of clients.
- Registered Substance Abuse/ Addiction Counselor
- Working towards completion of required internship hours
- Must bebilingualand comfortable infacilitatingall groups and classes in both English and Spanish,readingand writing
- Completion Certificate of Addiction Studies
- Valid drivers license and proof of insurance isrequired
- Must have knowledge of Microsoft Office 2000 and Excel.
- Experience working with Drug Medi-Cal clients (Adult and Youth)a plus.
- Understanding ofDrug Medi-Cal requirements/process a plus.
- Must have reliable transportation, as this position requires you to travel
- Must provide proof of up-to-date COVID-19 vaccinations including recommended doses in the primary series AND booster dose when eligible.
