SUMMARY

The Community Health Worker (CHW) is a field-based member of the Enhanced Care Management (ECM) team who has lived experience in the ECM patient community and serves as a liaison between the patient and the healthcare system. The CHW is responsible for helping patients and their families navigate and access community and healthcare services to adopt healthy behaviors.

WHERE YOULL WORK

This position ishybrid. Work from home and go into the field (Indio). Travel is requiredto conduct in-person member visits within designated areas. You will have full control over your schedule when meeting members. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:30 am to 5:00 pm.

WHAT YOULL DO

    • Provide support to patients who are difficult to engage and/or who have cultural or linguistic barriers to care via field-based contacts including accompaniment to appointments
    • Assist the Registered Nurse Care Manager, Behavioral Health Care Manager and Care Coordinator in panel management by performing delegated activities as assigned
    • Assist the Enhanced Care Management (ECM) team in engagement efforts of eligible patients in the clinic and within the community
    • Assist patients on navigating the healthcare system and community resources
    • Perform follow-ups by phone and in person with all eligible and enrolled ECM patients
    • Help patients successfully participate in medical and/or behavioral health care and overcome barriers to care
    • Update and provide information on patient barriers to the multi-disciplinary team and providers
    • Engage with patients, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as motivational interviewing) and promotes collaboration between the patient and the medical/behavioral team
    • Collaborate on patient care plans with ECM team members
    • Participate in weekly systematic and ad hoc case reviews
    • Consult with the RN Care Manager and/or the Behavioral Health Care Manager before taking any action that is clinical in nature
    • Accompany patients to office visits, as needed, and in the most accessible setting, within health plan guidelines
    • Perform other duties and projects as assigned

    WHO YOU ARE

    Sensory Requirements

    • Fluent in English (written and verbal), Bilingual in Spanish
    • Ability to communicate clearly in-person, by phone, and electronically
    • Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation
    • Ability toidentifyproblems and use logic and related information to develop and implement solutions
    • Excellent organizational skills and attention to detail
    • Commitment tomaintainingpatient confidentiality and adhering to ethical standards in healthcare practice

    Physical Activity

    • Ability to lift, carry, push, or pull up to 2025 pounds(e.g., laptop bag, forms, mobile equipment)
    • Ability to climb stairsor navigate uneven terrain in community and home environments
    • Ability to bend, reach, and conduct in-person visits in non-traditional environments
    • Must be able to remain in a stationary position
    • Must be able to move around the office and/or travel throughout community
    • Ability tooperatea vehicle and travel to meet with members around the community; attend meetings and events asrequiredor requested

    Environmental Conditions

    • Work may occur in homes, shelters, outdoor settings, hospitals, or community organizations, which may include exposure to pets, smoke, odors, clutter or unsanitary condition, and varying temperature conditions
    • Ability tomaintainprofessionalism and safety in diverse environments
    • Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures

    Technology Use

    • Frequent use of computers, keyboard, and handheld/mobile devices
    • Ability to type for extended periods
    • Competent with computers, email, virtual platforms, and Microsoft Office based programs

    WHAT YOULL NEED

    • Diploma/GEDrequired, associate degree preferred
    • Minimum1+ years of experience in healthcare, social services, or care coordination
    • Current and valid Drivers License with a clean driving record
    • Reliable transportation and proof of auto insurance
    • Current BLS certification from the American Heart Association upon start date
    • Distraction-free home workspace with a secure internet connection
