Job Summary\n
Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities\n

- Coordinate patient care and discharge planning efficiently.
- Utilize Epic charting system for accurate documentation.
- Manage a case load of up to 25 patients, tailoring care to multidisciplinary needs.
- Engage in care coordination with healthcare management organizations (HMOs) and independent practice associations (IPAs).
- Facilitate communication and planning within interdisciplinary teams to ensure patient needs are met.
- Participate in care coordination, utilizing evidence-based guidelines to determine medical necessity.

Qualifications\n
- \n
- Two years of acute hospital experience as a Care Manager.
- Experience with care coordination, discharge planning, and assessing patient needs required.
- Proficient in using computerized documentation systems such as Epic.
- Familiarity with CMS, CPT coding, and HIPAA guidelines.
- Experience working with adults and geriatric patients in multiple healthcare settings.
- Required to have travel experience and ability to float between facilities.
