Job DescriptionAbout the Role:Aura One Hospice is looking for a compassionate and dedicated Certified Home Health Aide (CHHA) to join our team in Palm Desert, CA. Responsibilities:
- Provide personal care services to hospice patients in the home setting, including bathing, grooming, and hygiene assistance
- Assist patients with mobility, transfers, and ambulation as directed by the care plan
- Monitor and document patient condition changes and report observations to the supervising nurse
- Support patients with light housekeeping, meal preparation, and activities of daily living (ADLs)
- Deliver compassionate, patient-centered care in alignment with hospice philosophy and end-of-life care standards
- Communicate effectively with interdisciplinary hospice team members, patients, and families
- Maintain accurate documentation and adhere to care plan guidelines
Requirements:
- Valid California Certified Home Health Aide (CHHA) certification required
- Prior experience in hospice, home health, or palliative care preferred
- Current CPR/First Aid certification
- Reliable transportation and valid driver's license
- Strong interpersonal skills with a compassionate, patient-focused demeanor
- Ability to work independently and as part of a collaborative care team
- Comfortable working in home environments with patients facing end-of-life care needs
