Minimum Qualifications

- Active California Registered Nurse (RN) License
- Minimum of 2 years of recent Acute Care Hospital Case Management experience
- Recent experience performing:\n

- Care Coordination
- Discharge Planning
- Utilization of community and post-acute resources
\n
\n
\n
- Acute Care Hospital Case Management experience is required
- Strong communication, organizational, and critical-thinking skills

Key Responsibilities
\n

- Coordinate patient care plans and discharge needs throughout hospitalization.
- Conduct comprehensive assessments to identify barriers to discharge and continuity of care.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop effective care plans.
- Facilitate timely discharges and appropriate transitions to post-acute care settings.
- Educate patients and families regarding treatment plans, resources, and discharge instructions.
- Advocate for patients while ensuring quality, cost-effective care delivery.
- Maintain accurate and timely clinical documentation.
- Support compliance with hospital policies, regulatory requirements, and quality standards.
