PT - Home Health

\n

Job Details:

\n

• Monday through Friday shift from 8:30 AM to 5:00 PM
\n • Home health care setting treating patients in homes, assisted living facilities, senior apartments, and board and care homes
\n • See 3 to 5 patients per day depending on visit type, with up to 2 Start of Care visits per day or 3 to 4 evaluations in various combinations
\n • Start of Care visits are weighted at 3.75 to 4 hours total including travel, documentation, and case management
\n • Treat a variety of diagnoses including orthopedic, cardiology, pulmonary, oncology, neurology, geriatric, and amputations - no pediatrics or hospice
\n • Supervise up to 2 PTA/COTAs
\n • 1 weekend shift per month requested but not required
\n • Holiday work is required with flexibility available if time off is requested in advance
\n • Robust 3-week orientation process with online learning and field precepting - solo work expected by end of 2nd or 3rd week
\n • Must be able to work in home environments that may be cramped, hot, cold, or cluttered
\n • Scrubs are not provided but recommended - dress code is business professional with closed-toe shoes required; no jeans, tank tops, or shirts with logos
\n • EMR system: Homecare Homebase (HCHB) experience preferred; Home Health experience including OASIS required
\n • 2 years of experience in a healthcare setting required; community or home health experience preferred
\n • Valid California PT license required
\n • Current driver's license and proof of automobile insurance required
\n • Required certifications: BLS (AHA) and NPI number
\n • Good oral, written, and organizational communication skills required
\n • Mileage is billable per GSA guidelines beginning after departure from first assignment - travel from housing to first visit does not qualify for reimbursement

This home health organization operates multiple service teams covering an extensive service territory, primarily supporting patients discharged from local hospitals and community physicians. The facility provides therapy services across a wide range of diagnoses and care settings, with a strong emphasis on continuity of care and efficiency. Physical therapy is managed by an RN Manager with additional support from a Regional Rehab Leader and Educator for therapy-related guidance.
