Job Description

Job Description

Physician Assistant

Hand & Microsurgery – San Francisco, CA (Lower Pac Heights • Mill Valley)

About Us

Hand and Microsurgery Medical Group is an independent, physician-owned practice devoted exclusively to hand and upper extremity care. Founded by Dr. Leonard Gordon, former Chief of Hand Surgery at the University of California, San Francisco, the practice is led day to day by his son, Dr. Joshua Gordon, who trained in orthopaedic surgery at the University of Pennsylvania and completed fellowship training in hand and microsurgery at the University of Washington, home to the only Level I trauma center in the region. Combined, the two bring nearly fifty years of clinical experience to the practice. Much of our staff has been with the practice for many years, a reflection of the kind of place this is to work.

The Role

We're hiring an experienced Physician Assistant to work closely with Dr. Gordon, managing independent clinic days and first assisting in surgery. No call, no weekends, a tight-knit team, and direct, day-to-day access to the physicians you work with, not layers of hierarchy. This is a clinic-only role, ideal for a PA who enjoys focused visits, clear care plans, and working autonomously within a small practice, with real opportunities to keep learning along the way.

Schedule & Setting

  • Work Locations (in person):
    • Lower Pacific Heights: 2299 Post St, San Francisco, CA 94115
    • Mill Valley: 141 Camino Alto, Mill Valley, CA 94941
  • Clinic hours: Monday–Friday, 8:00 AM–5:00 PM (no weekends, no on-call)
  • Schedule: A structured weekly rotation across both locations, including independent clinic days at Mill Valley, and time at Lower Pacific Heights first assisting in surgery and running an additional clinic day to help meet growing patient demand.
  • Support: On-site imaging, casting/splinting, and DME; close surgeon collaboration; modern workflows

Primary Responsibilities

  • Obtain HPI, perform focused orthopedic exams, review relevant PMH, and formulate diagnoses and comprehensive treatment plans.
  • Order and interpret diagnostics (in-office X-ray; referrals for MRI/CT; labs as indicated).
  • Perform therapeutic procedures (e.g., joint/soft-tissue injections, basic suturing, wound care).
  • Apply and manage splints/casts; fit and dispense DME appropriately.
  • Independently manage a full patient panel for new, follow-up, and post-operative visits, escalating complex or surgical cases to the attending surgeon as needed.
  • Document clear plans and progress notes; coordinate seamless follow-up and continuity of care (including surgical referrals when indicated).
  • Counsel patients on rehab, activity modification, DME use/care, medication adherence, and preventive strategies.

Minimum Qualifications

  • Education: Graduate of an ARC-PA–accredited PA program.
  • Certification/Licensure:
    • NCCPA certification (current and maintained)
    • California PA license (Physician Assistant Board) by start date
    • BLS certification
    • NPI (meets Medicare billing requirements)
    • DEA license (or eligible)
  • Experience:
    • ≥ 4 years of clinical practice, specifically in orthopedic experience, required (hand/upper extremity experience strongly preferred); must be able to see patients and manage care plans independently
    • Outpatient orthopedics, urgent/injury care, or sports medicine
    • Confidence with injections, fracture management basics, and DME
    • Comfort with high-efficiency clinics and patient education

Preferred Experience

  • Prior experience in hand, wrist, or upper extremity orthopedics
  • Familiarity with post-surgical and fracture splinting
  • Comfort working independently in a small, physician-led practice.

Work Environment & Growth

  • Robust on-site imaging and procedures for decisive, same-day care
  • Opportunities for skill expansion (procedures, imaging workflows, peri-op coordination) with mentorship

Compensation & Benefits

  • Pay: Range of $180,000.00–$200,000 per year, commensurate with experience
  • Employment type: Full-time, exempt
  • Benefits: 401(k) with match, medical/dental/vision, PTO, life insurance, voluntary insurance options, professional development support, licensure reimbursement, malpractice insurance.
  • Relocation assistance available for qualified out-of-area candidates, details discussed during the offer process.
