About the Role:
Only Care Home Health is looking for a compassionate and dedicated LVN Case Manager to join our growing team in Pasadena, CA. In this role, you'll play a vital part in coordinating and delivering high-quality patient care right where it matters most — in the comfort of our patients' homes. Responsibilities:
- Coordinate and manage patient care plans for home health clients in compliance with physician orders and agency protocols
- Conduct skilled nursing visits including assessments, wound care, medication management, and patient education
- Collaborate with interdisciplinary care teams including physicians, therapists, and social workers
- Monitor patient progress and update care plans to reflect changes in condition or treatment goals
- Ensure accurate and timely documentation in compliance with Medicare, Medi-Cal, and HIPAA regulations
- Communicate effectively with patients, families, and caregivers to promote continuity of care
- Manage caseload and scheduling to ensure timely delivery of home health services
Requirements:
- Active and unrestricted LVN license in the state of California
- Prior home health or case management experience preferred
- Knowledge of Medicare/Medi-Cal home health regulations and documentation standards
- Strong clinical assessment and critical thinking skills
- Excellent communication, organizational, and time management abilities
- Reliable transportation and valid California driver's license with proof of insurance
- Proficiency with electronic health records (EHR) systems
