Western Geriatrics and Neurology, part of Brigade Health, is hiring compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality mobile primary care to older adults in the comfort of their homes, skilled nursing facilities, assisted living, and senior communities.

What You'll Do
- Provide mobile primary care visits to seniors
- Manage chronic and acute conditions
- Perform comprehensive assessments & care planning
- Collaborate with caregivers, facilities, and interdisciplinary teams
- Deliver compassionate, patient-centered care
- Maintain accurate EMR documentation
Qualifications
- Active NP or PA license
- 1+ years of primary care, geriatrics, home health, hospice, SNF, or senior care experience preferred
- Strong communication and clinical skills
- Valid driver's license & reliable transportation
