Responsibilities

- Coordinate and manage patient care plans across the healthcare continuum
- Assess patient needs and develop individualized care strategies
- Collaborate with physicians, nurses, social workers, and other healthcare professionals
- Facilitate discharge planning and ensure appropriate follow-up care
- Monitor patient progress and resource utilization
- Document patient care activities accurately in Epic EMR
- Float between like units based on facility needs

Requirements
\n

- Active California RN License
- Minimum 2 years of Case Management RN experience
- Acute Care experience required
- Epic charting experience preferred
- Strong communication, assessment, and care coordination skills
- First-time travelers accepted with clinical approval
