Description
\n As a Medical Social Worker, you will be responsible for assessing patient needs, developing care plans, and coordinating post-acute care services. You will work closely with a multidisciplinary team to ensure effective discharge planning and assist patients and families throughout the care process. This role requires strong communication skills and the ability to manage a caseload of 20-23 patients, making a significant impact on patient outcomes.

- CA state license required (No Pending License accepted)
