Requirements\n

- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid and currently active Physical Therapy license (license required if applicable)
- Minimum 1 year of physical therapy experience
- SNF or similar post-acute care setting experience preferred
- Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
- Strong communication, interpersonal, and problem-solving skills
- DOB and SSN may be required for consideration

- Responsibilities include performing patient evaluations, developing individualized treatment plans, and providing therapeutic interventions to improve strength, mobility, and functional abilities

- Patient and family education on therapeutic exercises, safe movement techniques, and home modifications is a key component of the role
- Accurate and timely documentation of assessments, progress notes, and treatment plans is required in accordance with facility and regulatory standards
