Job Title: Certified Occupational Therapy Assistant (COTA)
Location: Skilled Nursing Facility (SNF)
Job Type: Full-Time / Part-Time

Pay: $50-$65/hr.


Position Overview

We are seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team at our Skilled Nursing Facility. The therapist will be responsible for evaluating, planning, and implementing individualized treatment programs that help residents improve mobility, independence, and overall quality of life.


Key Responsibilities

  • Perform comprehensive evaluations of residents’ functional abilities and limitations.
  • Develop and implement personalized treatment plans tailored to residents’ needs and goals.
  • Provide direct therapy services to improve mobility, strength, balance, coordination, and daily living skills.
  • Collaborate with an interdisciplinary team including nurses, physicians, and other therapists to ensure optimal resident care.
  • Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards.
  • Educate residents, families, and caregivers on therapy plans, home programs, and safety strategies.
  • Participate in care plan meetings and contribute to resident-centered outcomes.
  • Adhere to federal, state, and facility policies and regulations.


Qualifications

  • Graduate of an accredited Physical Therapy or Occupational Therapy program.
  • Current and valid state license as a PT or OT.
  • Skilled in evaluation, treatment planning, and therapeutic interventions for geriatric populations.
  • Strong communication, teamwork, and documentation skills.
  • Experience in a Skilled Nursing Facility (preferred, but not required).
