Overview of the Primary Care Physician Role
In the role of Primary Care Physician in Visalia, you will provide comprehensive outpatient medical services to a varied group of veteran patients. Your responsibilities will include preventive care, management of chronic conditions, and patient-centered services within a collaborative and well-equipped setting. You will have the freedom to cultivate your patient relationships and practice, backed by a skilled clinical team and administrative support.
Key Responsibilities
- Deliver thorough direct patient care, including taking medical histories, conducting physical examinations, managing medications, performing diagnostic evaluations, planning treatments, and utilizing clinical reminders.
- Lead team-based care for assigned veteran panels, employing tools such as registries to identify and meet both individual and population health needs, ensuring timely access and evaluations.
- Enhance team performance, oversee compliance with My HealtheVet, and provide care through VA Video Connect as necessary.
- Operate within the full scope of your clinical privileges while engaging in health education, wellness coaching, and patient involvement.
- Facilitate effective communication that fosters seamless coordination among team members, patients, and their support networks.
