Do you have leadership experience and a passion for helping individuals with intellectual and developmental disabilities live safe, meaningful, and independent lives? Home of Guiding Hands (HGH) is looking for a compassionate and organized Case Manager to join our Residential Services team.
In this role, you'll support the daily operations of multiple residential homes, supervise staff, coordinate client care, and ensure high-quality services while making a lasting impact on the lives of the people we serve.
What You'll Do
- Supervise House Managers and Direct Support Professionals.
- Support the health, safety, and well-being of clients living in residential homes.
- Coordinate medical appointments, medications, care plans, and client documentation.
- Train, coach, and support new and existing staff.
- Monitor compliance with licensing and agency standards.
- Assist with scheduling, payroll review, budgeting, and facility audits.
- Participate in interdisciplinary team meetings and help develop Individual Service Plans (ISPs).
- Step in to provide direct care and support when needed.
What We're Looking For
- At least 2 years of experience supporting individuals with intellectual and/or developmental disabilities preferred.
- Previous leadership or supervisory experience.
- Strong communication, organization, and problem-solving skills.
- Comfortable using computers, tablets, and electronic documentation.
- Valid driver's license with a safe driving record and at least 2 years of licensed driving experience.
- CPR, First Aid, Pro-ACT, and Medication Administration certifications (or ability to obtain if required).
- Must be at least 18 years old and successfully complete pre-employment screening.
