Responsibilities:

Conduct initial evaluations and develop individualized treatment plans for patients with a variety of medical conditions and injuries

Provide physical therapy services, including exercises, stretches, and modalities, to promote patient recovery and independence

Collaborate with interdisciplinary healthcare team members to develop comprehensive treatment plans and ensure patient-centered care

Monitor patient progress, adjust treatment plans as needed, and report significant changes or concerns to the healthcare team

Maintain accurate and detailed records of patient treatment and progress

Participate in patient education and discharge planning to ensure patients are equipped with the skills and knowledge necessary for successful rehabilitation and independence
