Job Summary
We are looking for a Respiratory Therapist to support our Asthma Remediation and Prevention program in the Antelope Valley area (Palmdale, Lancaster). This field-based role travels to patient homes to conduct in-home respiratory and environmental assessments, identify asthma triggers, and support members and families with education and remediation planning. The Respiratory Therapist will document assessments and care activity in Pear Suite, our Community Supports EMR system, and will receive full training on the platform.
This role works closely with Soteria's Community Supports team to coordinate care and ensure members receive timely, well-coordinated support toward improved respiratory health outcomes.
Please note that this a per diem position and is need based!
Responsibilities
- Conducting in-home respiratory and environmental assessments for members enrolled in the Asthma Remediation and Prevention program
- Identifying home-based asthma triggers and recommending appropriate remediation measures
- Educating members and families on asthma management, trigger reduction, and self-care strategies
- Documenting assessments, care plans, and follow-up activity in Pear Suite (training provided)
- Coordinating closely with Soteria's Community Supports team to ensure continuity of care
- Tracking member progress and communicating updates to the appropriate care team members
- Maintaining compliance with CalAIM Community Supports guidelines, agency policies, and quality standards.
To succeed in this role, you should have:
- Current California Respiratory Therapist (RT/RCP) licensure
- Experience conducting patient assessments; home health or community-based care experience is a plus
- Comfort traveling to patient homes throughout the Antelope Valley area (Palmdale, Lancaster, and surrounding communities)
- Willingness to learn and use the Pear Suite EMR system (training provided)
- Strong communication and coordination skills to work effectively with the Community Supports team
- A professional, reliable, and self-directed work style with strong attention to detail
- Bilingual skills are a plus for enhanced member communication and care coordination
