Shift Type: Day shift, 8-hour shifts (between 8:00 AM - 5:00 PM), every other weekend, first to float
Hours per Week: 40
Candidate Pay Rate: $81 - $82 per hour

Roles & Responsibilities
- Conduct comprehensive initial assessments to determine patient care needs and develop individualized care plans.
- Coordinate discharge planning from admission through post-acute care, ensuring smooth transitions for every patient.
- Perform utilization review using Interqual criteria to verify medical necessity and appropriate level of care.
- Collaborate daily with physicians, nurses, social workers, and patients to deliver high-quality, patient-centered care.
- Manage a caseload of 15-20 patients across the hospital, addressing all payers, including uninsured and homeless populations.
- Document assessments and plans in Cerner, maintaining accurate and up-to-date records.
- Reprioritize tasks efficiently throughout the day to respond to the dynamic needs of the unit and patient population.
Qualifications
- Active California RN license (or ability to obtain before start)
- BLS certification required
- BSN required
- Minimum 2 years of Case Management/Care Coordination experience
- Recent prior Dignity Health (CA) experience required
- Proficient with Interqual criteria and Cerner EMR
- Must not have been directly employed by CommonSpirit (CHI/Dignity) within the past year
- Provide 2 supervisory references (one within 12 months, one within 3 years)
Education
- Bachelor of Science in Nursing (BSN) required
Knowledge & Skills
- In-depth knowledge of utilization review and discharge planning processes
- Familiarity with Interqual criteria for medical necessity with ability to train
- Strong patient assessment and care coordination skills
- Excellent communication, collaboration, and teamwork abilities
- Ability to manage a high-volume caseload and reprioritize on the fly
- Experience working with diverse patient populations, including uninsured and homeless
