Title: Speech Therapist II
\n Duration: 3 Months (Possibility to extend or convert)
\n Shift: Mon- Fri (8:30am -5pm)

\n Position Overview
\n We are seeking an experienced Speech Language Pathologist (SLP) to provide home health therapy services to geriatric patients. The Speech Therapist will assess patients for speech and swallowing-related therapy needs, establish treatment goals, and implement individualized care plans to improve communication and functional outcomes.
\n The ideal candidate will have experience in home health and geriatrics, with a strong clinical background across multiple diagnoses including neurology, cardiopulmonary, orthopedics, gastroenterology, and otolaryngology.
\n Key Responsibilities:
\n Evaluate patients for speech, language, cognitive, and swallowing impairments
\n Develop and implement individualized Speech Pathology treatment plans
\n Review patient medical history and perform clinical screenings
\n Establish therapy goals and discharge recommendations in collaboration with patients and families
\n Educate patients, caregivers, and families on therapy activities, equipment use, and home exercises
\n Perform ongoing re-evaluations and adjust treatment plans as needed
\n Collaborate with multidisciplinary healthcare teams
\n Maintain timely and accurate clinical documentation in compliance with regulatory standards
\n Manage multiple patients efficiently within a home health setting
\n Participate in continuing education and apply updated clinical practices
\n Assist with orientation and training of new staff and interns (Speech Therapist II responsibilities)
\n Conduct literature reviews and provide staff education/in-services when needed
\n Required Qualifications & Experience
\n Minimum 18 months of experience as a Speech Language Pathologist
\n Current California SLP License required
\n Current BLS Certification required
\n Experience treating geriatric patients required
\n Home health experience preferred
\n Strong knowledge in:
\n General Medicine
\n Orthopedics
\n Neurology
\n Cardiopulmonary
\n Gastroenterology
\n Otolaryngology
