A PACE (Program of All-Inclusive Care for the Elderly) center opening in the Stockton, CA area is seeking a Primary Care Physician to join its interdisciplinary care team.
Responsibilities:
Provide comprehensive primary medical care to a panel of PACE participants, most of whom are frail, elderly individuals with complex, multiple chronic conditions. Conduct medical evaluations, diagnose and manage acute and chronic conditions, and develop individualized treatment plans. Serve as an active member of the interdisciplinary care team (IDT), collaborating with nursing, social work, rehabilitation therapy, dietary, recreation, and personal care staff to build and update each participant's plan of care. Participate in regular IDT meetings. Coordinate participant care across settings, including center-based visits, home care coordination, and specialist or hospital referrals. Order and interpret diagnostic testing, prescribe and manage medications, and refer to specialty care as needed. Provide clinical oversight and collaborative support to Nurse Practitioners and other clinical staff as applicable. Maintain accurate, timely documentation in the electronic medical record. Support quality improvement initiatives and compliance with CMS and state PACE program requirements. Participate in an on-call rotation as needed.
Requirements:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred. Active, unrestricted California medical license, or ability to obtain prior to start. Current DEA registration. Experience with geriatric or medically complex populations preferred. Prior experience in a PACE program or similar interdisciplinary care model preferred.
