We are seeking a licensed Physical Therapist to join a reputable Skilled Nursing and Rehabilitation Facility in Roseville, California. This facility provides post-acute rehabilitation, long-term care, and memory care services to adult and senior patients. The Physical Therapist will play a key role in evaluating patients, developing individualized treatment plans, and delivering evidence-based interventions to support functional recovery and mobility.

Key Responsibilities

Perform comprehensive physical therapy evaluations and assessments

Develop and implement individualized plans of care based on patient needs and goals

Provide therapeutic interventions focused on mobility, strength, balance, and functional independence

Monitor patient progress and modify treatment plans as clinically indicated

Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and medical providers

Educate patients and families on treatment goals, progress, and home exercise programs

Maintain accurate, timely documentation in compliance with facility policies and regulatory requirements

Utilize evidence-based practice standards to support optimal patient outcomes
Qualifications

Active California Physical Therapist license in good standing

Prior experience in a Skilled Nursing Facility or post-acute setting preferred

Strong clinical assessment and treatment planning skills

Excellent communication and interpersonal abilities
