We are seeking an experienced Director of Clinical Operations who can independently manage the day-to-day operations of a Medicare-certified home health agency.
This is a hands-on leadership position requiring strong operational and clinical oversight. The ideal candidate will have significant experience working in a Medicare-certified home health agency and be capable of managing daily agency operations, clinical staff, patient workflow, OASIS requirements, and Medicare compliance with minimal direction from ownership.
Key Responsibilities
- Oversee daily agency operations from referral and intake through discharge
- Manage intake, referral review, and appropriate patient acceptance
- Coordinate staffing and scheduling of Start of Care (SOC) and ongoing visits
- Manage RNs, LVNs, PT/OT/ST, HHAs, and other clinical staff
- Monitor Start of Care, Resumption of Care, recertification, transfer, and discharge timelines
- Track hospitalizations and ensure timely and appropriate follow-up
- Oversee OASIS workflow, accuracy, and assessment timelines
- Monitor physician orders, Plans of Care, and outstanding clinical documentation
- Ensure compliance with Medicare home health requirements, including skilled need, homebound status, Face-to-Face Encounter requirements, certification, and recertification
- Ensure visits and clinical documentation are completed timely
- Manage missed visits, call-outs, staffing issues, and changes in patient status
- Lead case conferences and interdisciplinary care coordination
- Monitor clinical staff performance, productivity, and accountability
- Oversee QAPI, quality initiatives, and survey readiness
- Identify and resolve operational issues before they become compliance or patient-care problems
- Maintain effective systems for tracking patients, clinicians, orders, assessments, and required deadlines
Qualifications
- Significant experience working in a Medicare-certified home health agency required
- Home health leadership or operational experience preferred, including clinical management, administration, or oversight of day-to-day agency operations
- Strong working knowledge of Medicare Conditions of Participation, OASIS, Plans of Care, Face-to-Face Encounter requirements, certification/recertification, and home health clinical documentation
- Experience overseeing clinical staff, admissions, scheduling, patient census, documentation timelines, and interdisciplinary coordination
- Strong leadership, organizational, communication, and problem-solving skills
- Ability to independently manage daily operations, prioritize competing responsibilities, and follow issues through to resolution
