Position Overview:
The Director of Social Service functions to plan, develop, organize, implement, evaluate, and direct social service programs in accordance with current federal, state, and local standards, guidelines, and facility policies and procedures to ensure that the medically related emotional and social needs of each resident are met and maintained on an individual basis.
- Assist in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Assist, as directed, in the admission process; interviewing perspective residents, families or responsible parties.
- Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the residents.
- Work with emotional problems including assisting residents/family with anxieties and stress caused by illness and death, and the need for institutional and specialized care.
- Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
Qualifications:
- Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human service field (e.g., sociology, special education, rehabilitation counseling, psychology).
- Licensed Clinical Social Worker or equivalent license preferred, but not required.
- Minimum of 3 years related social services experience in a skilled nursing or long-term care setting, including aspects of discharge planning and psychosocial assessments.
