Job DescriptionWe are seeking compassionate, resourceful, and motivated Case Manager to support individuals experiencing homelessness through outreach, intake, resource navigation, and housing stabilization services throughout Central Orange County.
This is an excellent opportunity for professionals who enjoy building meaningful relationships, working in the community, and helping people take the next steps toward self-sufficiency and independence.
What You'll Do
- Conduct client intakes, assessments, and enrollments.
- Meet with clients in community and public settings throughout Central Orange County.
- Build rapport and establish trusting relationships with individuals experiencing homelessness.
- Connect clients to housing resources, benefits, healthcare, employment services, and community programs.
- Assist clients with completing applications and required documentation.
- Support clients as they transition into housing and work toward stability.
- Develop individualized service plans and track client progress.
- Maintain timely and accurate case notes and program documentation.
- Collaborate with community partners and internal support teams.
- Empower clients to achieve their personal goals while encouraging accountability and self-sufficiency.
What We're Looking For
- Experience in case management, homeless services, housing navigation, social services, behavioral health, or a related field.
- Strong communication, engagement, and relationship-building skills.
- Ability to work independently in the field and manage multiple priorities.
- Knowledge of community resources and supportive services preferred.
- Strong organizational and documentation skills.
- Comfortable working with diverse and vulnerable populations.
- Valid driver's license, reliable vehicle, and current auto insurance required.
- Mileage is reimbursed
Pay Details: $23.00 to $26.00 per hour
