 We are looking for a per diem Physical Therapist Assistant (PTA) with recent Home Health experience to visit patients in their homes in&nbsp; Perris, Murrieta, Lake&nbsp; Elsinore, Menifee and Hemet&nbsp; and surrounding cities. Apply your home health therapy experience and provide the high quality care to our patients. We thrive on our patient care in our communities! Responsibilities and Duties:

<li>Assess patient's condition under a PT during every visit and chart observations</li>
<li>Perform evaluation tasks, including vital signs</li>
<li>Follow prescribed treatments, following plan of care and assess progress of healing</li>
<li>Provide appropriate timely feedback to Physical Therapist and other professionals</li>
<li>Charting and documenting</li>
