Physical Therapist (PT)&nbsp; $45–$50/hour | 1099
Compass Palliative Care is seeking a compassionate and experienced Physical Therapist to provide in-person care to patients in Skilled Nursing and Assisted Living Facilities.
Responsibilities
<ul>
<li>Provide physical therapy evaluations, treatments, and individualized care plans focused on mobility, strength, balance, pain management, and quality of life</li>
<li>Monitor patient progress, maintain clinical documentation, and collaborate with facility staff and the healthcare team</li>
</ul>
Requirements
<ul>
<li>Active and unrestricted California Physical Therapist license</li>
<li>Graduate of an accredited Physical Therapy program</li>
<li>Experience with geriatric, medically complex, or chronic-care patients preferred</li>
<li>Strong communication skills and ability to work independently</li>
<li>Reliable transportation for in-person visits</li>
</ul>
