ABOUT US
Horizon Wound Care is a physician-led practice based in Seal Beach. Since 2024, under our Medical Director, Dr. Amir
Alexander Rouhani, we have brought wound care to patients where they are: skilled nursing facilities, PACE programs,
assisted living, homes, and clinics. We are busy and growing, including our work with a San Diego PACE program.
THE OPPORTUNITY
We are looking for Nurse Practitioners and Physician Assistants to provide mobile, bedside wound care across Los Angeles,
the San Fernando Valley, Orange County, and the Inland Empire.
There is plenty of work. Most providers start per diem, paid by the visit. If your schedule grows to full-time hours, we will talk
about salary.
WHAT YOU WILL DO
Assess and treat chronic wounds: pressure injuries, diabetic foot ulcers, venous and arterial ulcers, and surgical wounds
Perform debridement and apply advanced wound products, skin substitutes, and grafts, within your scope
Order diagnostics and recommend dressings, offloading, compression, and nutrition support
Document visits in the EHR, including measurements and photos
Collaborate with supervising physicians, facility nursing staff, PACE teams, and primary care providers
Teach patients and families how to care for their wounds
QUALIFICATIONS
Required
NPs: active California NP license, furnishing number, and AANP or ANCC certification
PAs: active California PA license and NCCPA certification
DEA registration, or the ability to get one quickly
Current BLS
Reliable car and a valid driver's license; daily travel within your region
Preferred
Wound care experience in long-term care, home health, or a wound clinic
WCC, CWS, or CWCN certification
Experience with debridement and grafts
New to wound care? If you are an experienced NP or PA ready to specialize, we will train you.
COMPENSATION
$125 to $175 per visit, based on experience and wound care knowledge
$120,000 to $180,000 a year once your schedule reaches full-time hours
Steady patient volume and room to add days
You choose your schedule
Training and mentorship from experienced wound care physicians
EMPLOYMENT TYPE
Per diem, part-time, or full-time
W-2, or 1099 if you have your own professional corporation (we contract with your corporation)
