Compensation:

Position Summary &amp; Responsibilities (SNF)

Evaluate and treat SNF residents
</li>
<li>
Document care; ensure Medicare/PDPM compliance
</li>
<li>
Collaborate with interdisciplinary care team
</li>
<li>
Support smooth facility startup and operations
</li>
</ul>
Requirements
<ul>
<li>
Active Physical Therapist (PT) license
</li>
<li>
SNF or post-acute rehab experience required
</li>
<li>
Knowledge of Medicare/PDPM and documentation standards
</li>
<li>
Ability to work in newly transitioned facility environment
</li>
</ul>
