Role Description This is a contractor role for licensed OTs, and COTAs. The clinician will evaluate patients, develop individualized treatment plans, and provide hands-on therapeutic interventions to support functional mobility, self-care, and overall independence. Daily responsibilities include documenting care in a timely and accurate manner, monitoring progress, adjusting treatment as needed, and educating patients, caregivers, and families on home programs and safety. The role involves collaborating with an interdisciplinary team, participating in case discussions, and contributing to program development to improve patient outcomes. The clinician will also support quality assurance initiatives and maintain compliance with all regulatory, ethical, and professional standards.

Qualifications

- Demonstrated proficiency in Patient Care and Rehabilitation, including assessment, treatment planning, and evidence-based interventions.
- Experience or training in Occupational Therapy, with the ability to address functional and daily living skills as appropriate to license (OT, COTA).
- Skills in Home Care, including preparing and instructing effective home exercise programs and educating patients and caregivers.
- Current licensure or certification as OT or COTA in the state of California, in good standing.
- Strong documentation, time management, and organizational skills, with attention to detail and regulatory compliance.
- Effective communication and interpersonal skills to work with diverse patients, families, and interdisciplinary team members.
- CPR certification and familiarity with standard safety and infection control practices.
- Ability to safely perform the physical demands of the role, including assisting with transfers and therapeutic activities.
- Previous experience in outpatient, home health, or community-based rehabilitation is preferred but not required.
