A More Personal Approach to Care

At Legacy Concierge, we believe every client deserves personalized, one-on-one care. As a Caregiver, you'll provide dedicated 1:1 care to one client at a time in the comfort of their home, typically for an 8-12 hour shift.

Unlike facility settings where caregivers are often responsible for multiple residents at once, our caregivers have the opportunity to focus on one client, build trust, understand their routines and preferences, and make a meaningful impact on their quality of life.

Key Responsibilities

Caregiver Responsibilities

  • Deliver personalized 1:1 non-medical care in the client's home.

  • Assist with Activities of Daily Living (ADLs), including but not limited to:

    • Personal care

    • Bathing

    • Dressing

    • Grooming

    • Toileting and incontinence care

    • Transfer assistance

    • Mobility support

    • Medication reminders

    • Proper hand hygiene

  • Assist with Instrumental Activities of Daily Living (IADLs), including but not limited to:

    • Meal planning and preparation

    • Light housekeeping

    • Laundry

    • Transportation (if applicable)

    • Grocery shopping

    • Companion care

    • Community engagement and activities

  • Complete daily care notes accurately and promptly using agency documentation systems.

  • Provide support to clients with a variety of conditions including, but not limited to:

    • Alzheimer's disease

    • Dementia

    • Parkinson's disease

    • Multiple Sclerosis (MS)

    • COPD

    • CHF

    • Post-operative recovery needs

    • Short- and long-term memory loss

  • Communicate promptly with the RN Care Ambassador and/or Care Coordination Team regarding changes in client condition, incidents, concerns, or other matters requiring escalation.

  • Maintain reliability, punctuality, and consistent attendance.

  • Demonstrate Legacy Concierge's core values and client-centered care philosophy.

  • Maintain confidentiality and comply with HIPAA regulations and company policies.

  • Foster respectful, positive, and supportive relationships with clients, families, and team members.

  • Utilize sound judgment and professionalism while working independently in a client's home.

Required Qualifications

  • Minimum of 5 years of professional caregiving experience, personal caregiving experience, relevant education, or equivalent hands-on clinical experience.

  • Current California Home Care Aide (HCA) registration in good standing.

  • Proof of negative tuberculosis screening or chest X-ray.

  • Demonstrated ability to provide high-quality care in a one-on-one private care setting.

  • Ability to work independently while following care plans and seeking clarification when needed.

  • Strong communication, documentation, customer service, and interpersonal skills.

  • Reliable transportation and ability to travel to client homes.

  • Ability to use smartphones, tablets, and electronic documentation systems.

Physical Requirements

  • Ability to stand, sit, bend, kneel, reach, and move throughout a shift.

  • Ability to safely assist with transfers, repositioning, and mobility support using proper body mechanics and assistive devices.

  • Ability to lift, carry, push, or pull up to 50 pounds.

  • Ability to safely operate durable medical equipment (DME) when applicable.

  • Ability to use technology including smartphones, tablets, electronic charting systems, and learning management platforms.
