Requirements:
• Active CA RN License
• 2+ years Acute Medical Care Management experience
• Recent Care Coordination & Discharge Planning experience
• Epic experience REQUIRED
• Previous Travel Experience REQUIRED (HARDSTOP)

Responsibilities:
• Patient Assessments & Care Coordination
• Discharge Planning
• Family Meetings & Treatment Planning
• Arrange DME, SNF Placement & Transportation
• Collaborate with interdisciplinary teams
• Work with HMOs, IPAs, and complex social cases

Pay Range: $2,500 – $3,400/week
