About the Opportunity
\n Join a growing medical group as a Nurse Practitioner or Physician Assistant providing primary care for nursing home residents and following patients through skilled recovery stays. Leave the 9-to-5 clinic behind: you set your own day within a 7am to 7pm visit window, with no fixed office hours. Working parents can pick up their kids midday and get back to rounding afterward.

Key Responsibilities
\n - Provide primary care for frail and elderly residents across an assigned group of nursing homes
\n - Lead advance care planning conversations, including POLST
\n - Coordinate care with families, facility teams, and specialists

\n

Qualifications
\n - Board-certified NP or PA with a DEA
\n - Geriatric experience a plus
\n - Ideally 3+ years of experience as NP or PA
\n - Adult/Geriatric NP certification, long-term care, or skilled nursing experience a plus

\n

Compensation & Benefits
\n - $140,000 to $155,000 annually, plus productivity bonus for visits above 260 per month
